Clunking Noise When Turning Steering Whee

When I turn my steering wheel in either direction I hear a "clunk"
noise. Also sometimes when I hit a bump. Another thing I
notice is (especially) when I hit a bump, my steering pulls one way,
"clunk" pulls the other way. I have called this my re-alignment..
hehhe...
I know I have a worn inner tie rod, could that be the culprit? If
so, are replacing them difficult on the 1990 Olds Delta 88?

IF YOU KNOW YOU HAVE A WORN TIE ROD THEN YES YOU SHOULD REPLACE IT FIRST BEFORE LOOKING FOR ANOTHER CULPRIT. THIS SHOULD BE DONE BY AN EXPERIENCED TECHNICIAN AS THE TIE ROD HAS TO BE THE CORRECT LENGTH OR THE ALIGNMENT IS TOTALLY JUNK.
